Chatham's Touchpoint Pediatrics Recognized as a Patient-Centered Medical Home

Patient-Centered Medical Home 2011 standards (most current for 2014) emphasize enhanced care through patient-clinician partnership.

The National Committee for Quality Assurance (NCQA) today announced that Touchpoint Pediatrics of Chatham, NJ and its physicians, Dr. Jennifer Shaw-Brachfeld of Chatham, Dr. Valerie Tom of Short Hills, Dr. Margot Kerrigan of Basking Ridge and Dr. Stuart Slavin of Whippany, has received recognition as a level three (the highest achievable level) Patient-Centered Medical Home 2011. 

This program identifies offices committed to evidence-based, patient-centered processes that focus on highly coordinated care and long-term participative relationships. The patient-centered medical home is a model of care that emphasizes care coordination and communication and has been proven to lead to higher quality and lower costs while improving the experience of care for both patients and providers.

The NCQA Patient-Centered Medical Home 2011 standards (most current for 2014) emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.

“The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork and technology to deliver quality care and improve health”, said NCQA President Margaret E. O’Kane. “NCQA’s PCMH 2011 Recognition shows that Touchpoint Pediatrics and its physicians have tools, systems and resources to provide their patients with the right care at the right time.”

To receive recognition, which is valid for three years, Touchpoint Pediatrics demonstrated the ability to meet the program's key elements embodying characteristics of the medical home.
